~~ 2018 Christmas Assistance Guidelines ~~

 

Only one application will be accepted for each household. The Head of Household is to apply and may only request help for people who permanently reside in the home. If unrelated individuals live in one residence and share expenses they will be documented as one family (No one under 18 years of age may be an applicant).

The Christmas Assistance consists of food for all in the household, and toys for children ages 12 and under. Please note that this is an application process. It is your responsibility to demonstrate your need for this assistance. Please be familiar with your income and expenses.

You must reside in the following school districts in order to receive assistance from our location: Lewisburg, Mifflinburg, Milton, and Warrior Run.

Applications will be completed in an interview with one of our social services staff members or volunteers.

Sign-ups for assistance will be on the following dates:

 

October 2, 4, & 10 from 10:00am until 2:00pm
October 3, 9 & 11 from 6:00pm until 8:00pm

 

Spanish Translators available:
October 2, 2018 10:00am until 2:00pm
October 9 & 11 from 6:00pm until 8:00pm

 

No Children Allowed during Applications


You MUST bring current documentation of the following. All documentation must be originals (no copies accepted):

1. IDENTIFICATION for EVERYONE - OF ALL AGES - in your household including Dates of Birth applicant must have photo I.D. Social Security Cards, or Birth Cirtificate for everyone else in the home.

2. Proof of ADDRESS IN YOUR NAME (HEAD OF HOUSEHOLD) including the zip code. Examples: Lease, Utility Bill, Signed letter from your landlord on letterhead, or Current Rent Receipt.

3. Proof of all household income (this will be copied for our records), financial help, and benefits (whether the benefit or help is received by you personally or is paid on your behalf) is required. Examples include: Determination Letter (from within the past six months) of Public Assistance with *food stamps, cash benefit, and rent benefit award amounts. Section 8 letter. Pay Stub. Unemployment Verification Statement. Personal Letter (dated with address and signature) describing the assistance provided to you by that person.*NOTE: Your food stamp access card is not sufficient documentation of financial assistance; you must have your SNAP notification - if you do not have this, you will need to obtain an official copy from your worker at the Dept. of Public Welfare.

4. Proof of all monthly expenses:

a. Proof of ENTIRE monthly RENT for your house/apartment (Examples: Lease, Signed letter from your landlord on letterhead, Canceled Check or Money Order Receipt, Award letter from Section 8 or Housing Authority (For Mortgage provide monthly statement or receipt.)

b. You must bring all utility bills (gas, electric, fuel oil). Bills which demonstrate your regular, monthly expenses will help to demonstrate your need for assistance, which is your responsibility.
